• Protect client confidentiality and privacy

When applicable, partners must comply with the Health Insurance Portability and Accountability Act (HIPAA).

Collaboration & Communication

Partners agree to:

• Participate in referral coordination
• Maintain professional communication with program staff
• Support collaborative care when appropriate

Information requested includes:

• Organization details
• Services provided
• Population served
• Type of collaboration requested
• Compliance and certification information

Once submitted, our team will review your application and contact you to discuss the next steps.

Organizations interested in partnering with Transpire Mental Health will complete the following steps:

1️⃣ Submit a Partnership Application
2️⃣ Participate in a partnership review meeting
3️⃣ Complete partnership documentation
4️⃣ Establish a referral process
5️⃣ Sign a Memorandum of Understanding (MOU)
